When you visit Only Choice Urgent Care & Spa, Please remember, even if you are on existing patient, to bring a copy of:
Your photo ID, Address, & Phone/Cell number
Your up to date Insurance Card (Policy Holder name & DOB
(Medicare & Medicare Supplement if you have Medicare)
Your PCP (Primary Care Physician)
List of all current medications & any allergies
To save time, you can download and complete the New Patient Packet and bring with you at the time of your visit.
